www.health-choice.nl. It governs the personal data collection of clients, potential clients and website visitors in accordance to the General Data Protection Regulation (GDPR).

Health-Choice collects and processes personal data for the sole purpose of being able to provide safe, responsible and trustworthy services (massage and/or personal training) in compliance with the Dutch legal requirements.
Health-Choice does not send newsletters or any other marketing or commercial campaigns.
Processing personal data:
Health Choice collects and processes personal information data for the following purposes:
1. To enter into client relation: when a client request a service basic personal data is required such as
name, telephone number, email and address.
This is to enable Health Choice to communicate with the client in the context of its services.
2. For financial processing: Health Choice requires your personal data to fulfil the legal requirements
for invoicing and payments.
Sensitive data:
Sensitive information constitutes a special category. Sensitive data are those relating to your medical and health condition. Health Choice collects information about your health in order to provide you with its services effectively.
It is Health Choice policy to collect only sensitive data that are reasonably necessary to provide safe massage and/or training.
Prior to the 1st massage or personal training session, clients will be required to complete a hard-copy health questionnaire. The questions have the sole purpose of providing a quick assessment of your general state of health and in particular if there are any precautions or contra-indications to consider. The assessment for rehabilitation or personal training is more extensive. This is to be able to set up a results-oriented program, based on your request and goals.
Health choice is authorised to process your personal data for the above purposes under the following conditions: if you have given your consent for such processing and/or if the processing is necessary in order to render the agreed services and/or to comply with a legal obligations.
Client information is not retained longer than strictly necessary or for accountability to external regulatory bodies.
Rights:
You have the right to:
– view, correct and delete your personal information, unless the erasure of such data is in conflict
with the legal requirements
– object to data processing. This could make it difficult or even impossible for Health Choice to
provide the services you require
Health Choice will not forward your personal information to third parties without your permission, unless required by law.

No personally identifiable information is collected when browsing the site.
Health-Choice does not use cookies or similar technologies and is compliant with EU regulations on online tracking.
The website contains links to third party websites. Health-Choice is not responsible for the privacy policies or security practices of those websites. You are encouraged to read their privacy statement before supplying any personal information to them.
Protection:
Health-Choice understands the importance of protecting personal information and suitable precautions are in place to prevent misuse or unauthorised access.
The hard-copy health questionnaire is accessed only by André Schutter, it is not digitalised and it is securely stored in a locked cabinet or restricted area. Electronic hardware is also secured in a locked or restricted area at all times.
